To ensure that you receive the most effective healthcare, we ask that you bring your insurance card and a current medication list to every appointment. If you have an insurance copay, it is due at the time of the appointment.

New Patients
  • Before we set up your first appointment, please verify with your insurance company that Dr. Schock is in your network.
  • When you set up a new patient appointment, you can expect a 10 minute conversation to make sure we get all relevant information into our system. Please have your insurance information available during this conversation. It usually takes about 2 weeks to get in for a new patient appointment.
  • PATIENT REGISTRATION FORMS should be completed and brought in 15 minutes prior to your first visit. If you have any questions regarding the forms, feel free to contact us at 815.229.9900. If you do not have access to a printer, we ask that you arrive at your scheduled appointment 30 minutes early to fill out the paperwork.
  • NOTE: We do not reschedule missed or cancelled new patient appointments.
Reschedule Policy

If you are unable to make your scheduled appointment, please call our office at least 24 hours in advance  to avoid a “No Show” fee.

Mulford Medical’s “No Show” policy is as follows:

  • Cancellations less than 24 hours in advance (unless an emergency).
  • Arriving 15 minutes past the scheduled appointment time.
  • After 3 “No Shows”, no further appointments will be scheduled.

This policy is to ensure all our patients timely and effective healthcare. We appreciate your understanding in this matter.
